A physical examination dated after August 15, 2018 and a current immunization record must be submitted to the Glenbard East Health Office for every incoming 9th grade student as soon as possible, and no later than August 1, 2019. Please be aware that incoming freshmen will require 2 doses of varicella or documentation by your physician of chickenpox disease. The state of Illinois mandates that the health physical must be submitted on the official Illinois Department of Public Health Certificate of Child Health Examination Form. The IHSA Sport Physical form cannot be accepted. Once your 9th grade student has submitted his/her physical examination and immunization record to the Health Office, and if he/she wishes to participate in a sport, a separate sport physical will not be needed, but you may wish to give a copy to your student’s coach. District 87 Board Policy states that failure to comply with the above requirements by the first day of school will result in the student’s exclusion from attending school.
