IMMUNIZATION REQUIREMENT FOR GRADE 12

• The Illinois Department of Public Health requires all students entering or continuing in Grade 12 to show proof of receiving two doses of the Meningococcal vaccine on or after their 11th birthday. Only one dose is required if the first dose was received at 16 years of age or older. Please have your student return proof of immunization to the Health Office no later than August 1st, 2019. Students who are not in compliance by the first day of school will be unable to attend class.
